Use of Predictive Modeling to Plan for Special Event Medical Care During Mass Gathering Events

Objectives:
In 2010, South Africa (SA) hosted the Fédération Internationale de Football Association (FIFA) World Cup (soccer). Emergency Medical Services (EMS) used the SA mass gathering medicine (MGM) resource model to predict resource allocation. This study analyzed data from the World Cup and compared them with the resource allocation predicted by the SA mass gathering model.
Methods:Prospectively, data were collected from patient contacts at 9 venues across the Western Cape province of South Africa. Required resources were based on the number of patients seeking basic life support (BLS), intermediate life support (ILS), and advanced life support (ALS). Overall patient presentation rates (PPRs) and transport to hospital rates (TTHRs) were also calculated.
Results:BLS services were required for 78.4% (n = 1279) of patients and were consistently overestimated using the SA mass gathering model. ILS services were required for 14.0% (n = 228), and ALS services were required for 3.1% (n = 51) of patients. Both ILS and ALS services, and TTHR were underestimated at smaller venues.
Conclusions:The MGM predictive model overestimated BLS requirements and inconsistently predicted ILS and ALS requirements. MGM resource models, which are heavily based on predicted attendance levels, have inherent limitations, which may be improved by using research-based outcomes.

This study examined the mediating role of loneliness (assessed by self-report at Time 2; Grade 6) in the relation between early social preference (assessed by peer report at Time 1; kindergarten through Grade 3) and adolescent anxious/depressed symptoms (assessed by mother, teacher, and self-reports at Time 3; Grades 7–9). Five hundred eighty-five boys and girls (48% female; 16% African American) from three geographic sites of the Child Development Project were followed from kindergarten through Grade 9. Loneliness partially mediated and uniquely incremented the significant effect of low social preference in childhood on anxious/depressed symptoms in adolescence, controlling for early anxious/depressed symptoms at Time 1. Findings are critical to understanding the psychological functioning through which early social experiences affect youths' maladjusted development. Directions for basic and intervention research are discussed, and implications for treatment are addressed.

Objective: Gender appears to have a significant impact on the prevalence, age at onset, symptoms and outcome of schizophrenia. This study examines gender effects in a population of familial schizophrenic patients in Ireland.
Method: Families with two or more siblings suffering from schizophrenia, as defined by DSM-III-R were ascertained in Ireland. The final sample comprised 169 siblings from 80 families. Siblings were interviewed using the Structured Clinical Interview for DSM-III-R (SCID), the Scale for the Assessment of Negative Symptoms (SANS) and the Strauss-Carpenter Levels of Functioning Scale. The difference between males and females for various clinical features were calculated.
Result: There was a marked excess of affected males (65% male and 35% female). When the excess of male subjects was taken into account there was no significant excess of same-sex as compared to opposite-sex pairs. There were no significant difference between males and females for age at onset, age at first admission, symptoms or level of outcome.
Conclusion: The excess of males and the lack of gender differences for clinical features found in this study may, in part, be due to the narrow diagnostic criteria used. Alternatively, at least some of these findings may be specific to this Irish sample. Further research is a need to see if these findings can be replicated in other countries.
